One to One Health enhances health services and proves ROI with Jotform Enterprise

One to One Health is a healthcare organization that needed a better way to manage patient paperwork, surveys, and behavioral health forms across large-scale employers and government agency clients. They turned to Jotform Enterprise after finding that their existing EMR-based survey tools were expensive, inflexible, and hard to customize, which limited response rates and made patient feedback harder to capture.

With Jotform Enterprise and Jotform Teams, One to One Health streamlined new patient registrations, satisfaction surveys, PHQ-9/GAD-7 assessments, prescription refills, and other forms, while automating delivery and data routing to providers. The results were significant: adherence rates increased from about 20% to over 50%, the organization gathered 38,511 form submissions, and it used patient-reported data to show strong ROI, including that 33% of Text Care patients would have gone to urgent care and 5% to the ER.
