State of Victoria uses trakCare to Improve Community Healthcare

The State of Victoria’s Department of Human Services selected InterSystems TrakCare (community configuration) to be deployed across 22 community health agencies serving 5.6 million people. Evaluated by a panel of more than 150 healthcare professionals, the solution needed to integrate with Victoria’s statewide healthcare integration architecture and address fragmented client records, duplicated data entry, and poor coordination between agencies.

TrakCare’s embedded integration and central case-management give community health staff fingertip access to client information, eliminating repeated demographic and clinical entry and improving coordination across agencies. Early results include measurable efficiency gains, better resource allocation, improved client experience and preventative health outcomes—demonstrated across multiple sites such as the Western Region Health Center’s 13 locations.
