South Devon Integrated and Informed Care with HealthShare

Torbay and South Devon NHS Foundation Trust (TSDFT) is an integrated care organisation serving around 375,000 residents (plus seasonal visitors) across a mixed urban–rural area. After merging acute and community services and bringing together 6,000 staff, the Trust faced a major challenge: more than 100 disparate IT systems created data silos that prevented clinicians from accessing timely, consolidated patient information needed for safe, coordinated care.

TSDFT implemented InterSystems HealthShare to build the South Devon Clinical Portal — a clinician-led, single‑sign‑on platform that aggregates records from hundreds of systems. Piloted with the Heart Failure Team, the portal delivers real-time alerts, reduces duplication, improves handovers between acute and community teams, and empowers patients to view and update their care plans. Go‑live in December 2016 has accelerated roll-out to all clinicians and driven measurable improvements in communication, care coordination and patient engagement.
