Rhode Island Quality Institute (RIQI) used an ONC grant to pilot Designee Alerts, a service that sends clinical event notifications to patients’ family and friends to improve transitions of care, engagement, and coordination. The challenge was extending provider-focused alerts to this “third leg” of care while addressing privacy concerns and a burdensome designee enrollment process that limited adoption.
RIQI leveraged its CurrentCare statewide HIE, built on InterSystems HealthShare, to deliver real-time admission/discharge/transfer alerts via email or text to designated contacts and is developing a standalone mobile app to simplify enrollment and avoid giving designees full record access. Early results show reduced confusion at care transitions, faster intervention in cases such as opioid overdose (preventing “treated and streeted” outcomes), improved patient and family satisfaction, and potential downstream reductions in readmissions and costs.
