PAML (Pathology Associates Medical Laboratory) is one of the largest reference labs in the U.S., serving more than 100 hospitals, 25,000 clients and hundreds of thousands of patients each year. Rapid growth exposed limits in their legacy MultiValue billing system: nightly processing regularly exceeded the lab’s four-hour window, staff were delayed each morning, and PAML had to pause taking on new billing services.
To fix this they migrated the billing application from D3 to InterSystems Caché, converting roughly 1,500 routines and retaining MultiValue compatibility to minimize rewrites. The port allowed them to identify and optimize slow queries and scale to a distributed architecture—end-of-day processing dropped from 4–6 hours to about 1 hour and end-of-month from ~47 hours to 6 hours—restoring productivity, enabling new business, and creating a platform for Web services, embedded reporting and future growth.
