Northwell Health, one of the largest private health systems in the U.S. with 23 hospitals, 655 outpatient sites and 18,500+ physicians, faced a fragmented IT landscape from multiple EMRs across acquired organizations. That made it difficult to aggregate clinical data, coordinate care across settings, identify cohorts for population health and trials, manage risk, and deliver the right information into clinicians’ workflows.
Northwell deployed InterSystems HealthShare to build a unified health record—now holding over 1 billion data points for about 8 million patients—and piloted cross‑system sharing with Healthix. Using HealthShare as the backbone and a rule‑based care coordination app (Care Tool), Northwell delivered real‑time alerts, identity management and APIs for vendor integration, and achieved measurable gains: 6% fewer valve‑replacement readmissions, 18–28% more patients discharged home, reduced infections, up to 56% greater use of in‑network home care, faster innovation, and improved patient and clinician experience.
