Middle East Hospital Group cuts 30-day readmissions to 13% with InterSystems

A private hospital group in the Middle East sought to enable patients to avoid hospital admission by receiving acute care at home. Their challenge was to support this new model with up-to-date clinical and demographic information. To meet this need, they worked with InterSystems.

InterSystems implemented a solution using HealthShare Health Connect to receive data from in-home monitoring devices via a smartphone app. This data, reported in FHIR format, was integrated with the hospital's EMR and other systems. The results were significant, with 7-day readmission rates dropping to 4% compared to a national average of 8%, and 30-day readmission rates falling to 13% versus 20% for traditional in-hospital care.
