Greater Houston Healthconnect Coordinates Care Delivery for Millions of Texans

Greater Houston Healthconnect is a not‑for‑profit health information exchange that coordinates care for more than eight million people across 24 counties in the Greater Houston area. Its mission is to connect the region’s top health systems, hospitals and physicians, but doing so was complicated by a wide variety of top‑tier, homespun and legacy EMRs and an interoperability platform that could not scale with growing membership and data volumes.

Greater Houston Healthconnect migrated to InterSystems HealthShare with implementation support from J2 Interactive, creating a unified health record that connects roughly 70% of regional physicians and 95% of hospitals. The platform enables reliable information sharing, real‑time notifications and secure messaging, improving quality, efficiency and patient safety, bolstering disaster preparedness (as during Hurricane Harvey), and supporting initiatives to reduce readmissions and advance value‑based care.
