Gila River Health Care (GRHC) is a not‑for‑profit health system serving American Indian communities in Arizona, including a critical‑access hospital and multiple clinics. Facing a complex chronic‑care challenge, GRHC sought better diabetes case management by getting timely, actionable patient data to primary care teams to support the Chronic Care Model and improve control of A1C, blood pressure, LDL cholesterol and smoking cessation.

GRHC implemented InterSystems DeepSee—embedded real‑time business intelligence that integrates seamlessly with the Indian Health Service EHR (built on InterSystems CACHÉ)—and piloted it on 200 patients.
