Coordinate My Care (CMC) is a London-based service, born at the Royal Marsden, that records and shares patients’ end-of-life and urgent care wishes across multiple health and care providers. It was developed to tackle the mismatch between where people want to die (mostly at home) and the reality that nearly half die in hospital, creating a need for shared, accessible care plans during out-of-hours and emergency situations amid growing financial pressures on the NHS.
Working with InterSystems HealthShare, CMC delivers a web‑based, mobile-friendly urgent care plan that clinicians and patients can view and update; alerts notify 111, ambulance, out-of-hours and GP services and the plan appears in EMIS workflows. Deployed in over 1,000 GP practices with 35,000+ plans, CMC has helped 78% of patients die in their preferred place, reduced unnecessary hospital admissions and saves about £2,100 per patient (around £16.8m/year in London and a projected ~£556m nationally), improving both care quality and operational efficiency.
