InterCommunity Health Network Coordinated Care Organization (IHN‑CCO), part of Samaritan Health Plans, manages Medicaid care for Benton, Lincoln and Linn counties in Oregon and was charged with integrating behavioral, mental and physical health, shifting to fee‑for‑value, and addressing social determinants of health. Providers reported poor visibility into patient encounters between visits, creating care gaps and limiting the CCO’s ability to measure quality and control costs.
IHN‑CCO implemented InterSystems HealthShare to create a unified community health record, prioritizing connections to major hospitals, high‑volume clinics, behavioral health and dental providers, and linking to national exchanges where needed. The consolidated record now covers about 56,000 members, gives clinicians cross‑provider encounter visibility at the point of care, supports quality and payment reporting, and lays the groundwork to add social determinants of health and analytics to better identify and manage high‑risk patients.
