Stephanie Launches a Digital Patient Registration Process

Family Centers, a private nonprofit human service agency and healthcare clinic, needed to digitize patient registration and consent workflows for its medical, dental, and behavioral health services when it rapidly moved to virtual care. Working with Intellistack’s Formstack platform, the team also needed a way to collect insurance, demographic, and reporting data from clients who often lacked easy access to technology.

Intellistack implemented a simple digital registration process with online forms and e-signatures that clients could complete on phones, laptops, or desktops without downloading anything. The result was a more efficient workflow, easier completion for clients, and about 10% more data captured than before, including information that had previously been missed on paper forms.
