Exploring the successful transformation journey from disjointed information to triangulated safety, experience, clinical effectiveness and workforce information

County Durham and Darlington NHS Foundation Trust, one of the largest integrated healthcare organisations in the country serving over 650,000 people with a workforce of more than 7,000, struggled with fragmented quality and compliance data coming from multiple sources and at different times. The Trust engaged InPhase and its Oversight platform to build a single source of intelligence so clinical and nursing leaders could drill into ward- and team-level performance, identify exceptions quickly, and understand combined quality measures.

InPhase implemented Oversight to integrate data from its own suite (including 20 health apps) and third-party systems such as Datix, Perfect Ward, EPR and data warehouses, enabling rapid triangulation and visualisation of quality across wards. The solution delivered heat maps of monthly quality indicators, ward-to-board assurance, and one version of the truth that allows access to ward- or team-level data in a couple of clicks; the Trust’s Quality Committee now actively uses mined Perfect Ward data to target improvement and drive continuous assurance with InPhase.
