MercyOne PHSO, a large Midwest Accountable Care Organization managing over 310,000 patients across 400+ service locations and 196 participant organizations, faced fragmented data from 100+ clinical systems (including 15 different EHRs), decentralized care workflows, and limited real-time patient visibility that hindered patient engagement and value-based care. To address these challenges MercyOne partnered with Innovaccer and adopted the Innovaccer Health Cloud / Data Activation Platform to build a unified, event-driven 360° patient profile and automate care-team assignments and workflows.
Using Innovaccer’s platform, MercyOne integrated payer claims, ADT feeds, EHR data, scheduling and billing to enable live care plans, automated patient assignment, and real-time health coach scheduling; this drove measurable improvements including a 7.14% reduction in 30-day readmissions, millions in shared savings, site wellness visit completion rates above 70%, and health coach activity averaged 95.7 new engagements per coach per month. Innovaccer’s solution also helped MercyOne deliver over $70M in reduced healthcare expenditures historically and improved care-team performance tracking across the network.
