MercyOne PHSO, a large Midwest Accountable Care Organization managing over 310,000 patients across 400+ service locations and 196 participant organizations, faced fragmented data from 100+ clinical systems (including 15 different EHRs), decentralized care workflows, and limited real-time patient visibility that hindered patient engagement and value-based care.
