Locking Closed-loop Referrals With Seamless Community Resource Data Integration

Elevate Health, an Accountable Community of Health serving Pierce County, Washington, needed to close referral loops and coordinate care across dozens of partner sites but was hampered by data silos, decentralized workflows, and patient information scattered across EHRs, ADT/HIE feeds, claims and CCDA files. To address these challenges they worked with Innovaccer using the Innovaccer FHIR Data Platform to standardize and aggregate data and to enable seamless connections between providers and community social resources.

Innovaccer implemented its FHIR Data Platform to unify patient records, enable automated referral management and mobile apps for community resource workers, and surface analytics and point-of-care alerts for care teams. As a result, Elevate Health achieved real-time alignment of patients with food, housing, financial and other social services, improved visibility into network performance, and closed referral loops across the community; Innovaccer’s platform has also been used to unify millions of records and drive measurable savings industry-wide (3.8 million records and over $400M in reported savings).
