Improving care management through CRM technology

Largest Healthcare Company, headquartered in Englewood, Colorado and operating across 19 states with 105 hospitals, needed to modernize post-discharge care and CRM to improve community health. The system faced disparate data sources, no unified patient view, decentralized care coordination, time-consuming manual outreach, and poor patient communication, and therefore moved to the Innovaccer Health Cloud to strengthen transitional care management (TCM) and CRM capabilities.

Innovaccer implemented the Innovaccer Health Cloud—including Patient Engagement, Care Management, and its Data Activation Platform—to automate outreach and follow-ups, enable TCM protocols, unify records from clinics/claims/labs/pharmacies, and deliver point-of-care insights and omnichannel communication. As a result, Innovaccer helped reduce 30‑day readmissions from 15% in July 2020 to 5.6% in November 2020, closed care gaps, improved patient–provider communication, and generated $180,000 in savings over five months (with projected annual savings of about $432,000).
