How To Run Advanced Care Management With The Data Activation Platform

Mercy Accountable Care Organization faced fragmented clinical data across more than 100 disparate systems (including 15 different EHRs) spanning multiple states, which prevented a 360-degree patient view, hindered care coordination, and reduced effectiveness under value‑based care. To address these challenges, Mercy ACO engaged Innovaccer and implemented Innovaccer’s Data Activation Platform and the co-developed InCare care management solution.

Innovaccer ingested and normalized data from 400+ sources and 35 hospital ADT feeds, created a master patient index and 360° patient profiles, and enabled event-driven workflows and health coach scheduling through InCare. The solution drove measurable improvements: ED utilization per 1,000 fell 6.65%, 30‑day readmissions decreased 7.14%, annual wellness exam rates rose 31%, health coach engagements reached about 95.7 new engagements per coach per month, and primary provider services per 1,000 increased 14.26%.
