Hartford HealthCare, the largest health system in Connecticut managing tens of thousands of inpatient transitions and hundreds of thousands of ED visits annually, faced fragmented clinical data across dozens of ambulatory systems. With 36 unique ambulatory EHRs across more than 100 practice locations and inconsistent EHR versions/configurations, the network lacked the integrated clinical information and performance visibility needed to track quality measures and manage population health. To address this, Hartford HealthCare engaged Innovaccer and adopted Innovaccer’s Data Activation Platform.
Innovaccer’s Data Activation Platform used 200+ pre-built connectors to extract, standardize to a universal schema and CCDA format, and run 60+ data quality checks on disparate EHR data, creating longitudinal patient records and a normalized repository for analytics. As a result, Hartford HealthCare integrated 15 ambulatory EHRs across 34 practices, delivered over 1,000,000 C-CDA documents since February 2018, achieved network-wide interoperability, and generated data quality reports that improved visibility and enabled targeted population health and value-based care interventions.
