How To Effectively Reduce Ed Visits With A Data Activation Platform

Lowa Based Accountable Care Organization sought to reduce high emergency department (ED) utilization to lower costs and improve patient experience. To tackle fragmented data and limited visibility into frequent ED users, the ACO engaged Innovaccer and implemented Innovaccer’s Data Activation Platform.

Innovaccer deployed a Hadoop-based repository with 60+ EHR connectors to create Patient 360 records, applied risk stratification (HCC plus a social vulnerability index), and used InGraph and predictive analytics to identify high utilizers and enable targeted, automated outreach and preventive care programs. Innovaccer’s solution helped the ACO achieve a 6.65% reduction in ED utilization per 1,000, 95.7 new engagements per month per health coach, a 31% increase in annual wellness rates (with some sites >70% completion), and a 14.26% increase in primary care services per 1,000.
