How Mercyone PHSO Impacted Population Health Through Social Determinants Of Health

MercyOne sought to better understand and address social determinants of health across its large, regionally distributed population — over 310,000 patients under 20+ value‑based agreements across seven chapters and 870 service locations — but relied on paper‑based screening that was time consuming and administratively burdensome. To accelerate SDOH capture and drive population health improvement, MercyOne partnered with Innovaccer, using Innovaccer’s suite of solutions (including the Innovaccer Health Cloud) to unify patient data and enable a more data‑driven approach.

Innovaccer implemented an app‑based screening workflow and data unification tools that reduced documentation time from 2.5 minutes to 47 seconds per patient, enabled screenings for more than 7,000 patients (5,152 unique) across four clinics in a year, and resulted in 487 patients being identified and helped for social needs. Using Innovaccer’s platform, MercyOne rapidly identified food insecurity, transportation, and health literacy as top needs, cut administrative burden, and translated SDOH insights into actionable population‑health interventions.
