How Innovaccer and Emtiro Health Partnered to Build a Robust and Comprehensive Population Health Platform for Their Medicaid-Focused Provider Partners

Emtiro Health, a provider enablement organization focused on Medicaid populations, faced the challenge of building a population health platform that could handle highly fragmented clinical and non‑clinical data, capture social determinants of health (SDoH), and support complex care needs and AMH Tier 3/NCQA/LTSS requirements. To meet these needs, Emtiro partnered with Innovaccer and its Best‑in‑KLAS Data & Analytics Platform (Data Activation Platform), including applications such as InData, InGraph, and InCare.

Innovaccer’s DAP and integrated apps unified disparate patient data into longitudinal records, enabled SVI-driven insights and pre‑built SDoH assessments, integrated Healthwise for patient education, and automated transitional care management (TCM) workflows. As a result, Emtiro’s provider partners gained actionable analytics and care automation that improved coordination and outcomes—notably reducing pediatric Medicaid readmissions from 11.7% to 2.1% for patients with successful TCM protocols (Jan–Aug 2022).
