How an Iowa-Based Health System Reduced 30-Day Readmissions and Generated Millions in Shared Savings with Innovaccer’s Automated Transitional Care Management Protocols and Workflows

Lowa-Based Health System faced rising 30-day readmissions, fragmented patient data across hundreds of disparate systems, and manual, decentralized care-management workflows that hindered timely post-discharge follow-up.
