How an Iowa-Based Health System Reduced 30-Day Readmissions and Generated Millions in Shared Savings with Innovaccer’s Automated Transitional Care Management Protocols and Workflows

Lowa-Based Health System faced rising 30-day readmissions, fragmented patient data across hundreds of disparate systems, and manual, decentralized care-management workflows that hindered timely post-discharge follow-up. To address these challenges, the health system adopted Innovaccer’s Data Activation Platform and leveraged Innovaccer’s automated Transitional Care Management (TCM) protocols and workflows to standardize outreach, create unified longitudinal patient records, and enable real-time care coordination.

For Medicare patients between Jan–May 2022, patients with successful Innovaccer-enabled TCM outreach had a 9.9% 30-day readmission rate versus 14.6% without outreach (10.4% when a 14‑day PCP follow-up was completed), avoided $890K in readmission costs in that period, and produced an estimated $2.3M annualized impact.
