How a Nebraska-based CIN Enhanced Care Coordination with a FHIR-enabled Data Activation Platform

Leading Nebraska-based Clinically Integrated Network Company needed to unite 3,600 providers across 36 locations to coordinate care for 200,000 attributed lives but struggled with data silos across 15+ payer contracts, clinical variation, decentralized care coordination, and the lack of a single 360° patient view. To address these challenges, the network partnered with Innovaccer and adopted Innovaccer’s FHIR Healthcare Data Platform.

Innovaccer aggregated EHR, claims, pharmacy, hospital, clinic, and lab data into unified patient records, added analytics, dashboards, real‑time decision support, automated workflows and point‑of‑care alerts, and populated registries for 75 care coordinators. As a result, Innovaccer helped the network standardize protocols from over 75 to about 20, deliver a consolidated 360° operational view, automate clinical and claims ingestion, and improve visibility into contract performance, episode costs, and care coordination across the network.
