How a Leading Healthcare Network Reduced Readmissions and Length of Stay (LOS) With Post-operative Care Optimization

Leading Healthcare Network Company, a not-for-profit Midwest-based system with more than 750 beds and 1,000+ affiliated physicians, sought to revamp post-operative care to reduce excess 30-day readmissions and length of stay under HRRP.
