Leading Healthcare System faced fragmented data across multiple EHRs that undermined risk stratification and coding accuracy. To solve this, the Leading Healthcare System partnered with Innovaccer and deployed Innovaccer’s Healthcare AI Platform and point-of-care physician engagement solution (InNote) to unify data and surface coding and care‑gap insights directly within clinicians’ workflows.

Innovaccer integrated clinical, claims, and administrative data into a Patient 360 view and used InNote to prompt physicians to reassess and capture HCCs at the point of care, automating manual processes and closing gaps. The work produced a 70% improvement in coding accuracy, 8,572 Medicare Advantage coding gaps closed, $27 million in estimated gross benefits (including about $14M in recaptured HCCs and a $12M net value), and a 20–30% reduction in physician time spent on coding.
