OSF HealthCare identifies high risk patients for palliative care initiative

OSF HealthCare, an integrated health system with 11 acute care facilities and two nursing colleges, launched a system-wide initiative to expand palliative care and advanced care planning (ACP). Their challenge was to identify high-risk patients, engage them and their families in ACP, and measure care costs and alignment with patients’ expressed wishes across disparate data sources.

Using Informatica PowerCenter to integrate claims, registry and EMR data, OSF built a 360-degree view of high-risk patients and targeted outreach to initiate ACP conversations. Within three quarters they connected with 1,000 patients, enabled daily actionable reporting, and achieved better alignment of care with patient goals—resulting in improved quality and satisfaction and reduced costs.
