Hackensack Meridian Health, New Jersey’s largest health system (23 hospitals, ~36,000 team members and 7,000+ physicians), faced fragmented patient data after rapid growth through mergers and acquisitions. Multiple EMRs and credentialing systems made it difficult to identify patients across the network, track complete episodes of care, and give clinicians fast, trusted access to the right records while the system moved toward a centralized EMR.
Working with Infoverity, the system deployed Informatica Multidomain MDM and Informatica Enterprise Data Catalog to master patient data (including Epic/Clarity) from 15+ sources, standardize and route reconciled records back to disparate EMRs, and capture metadata lineage. In four months they consolidated records from 6.5 million to 3.2 million (a 49% reduction), connected siloed encounters across the full care continuum, and vastly improved data discovery, governance, and analytics to support better patient outcomes.
