Case Study: Capital District Physicians' Health Plan achieves expanded rural access and 24/7 virtual care with Included Health

CDPHP provides access to high quality healthcare for all their members, from the big city to the most rural towns

Capital District Physicians' Health Plan (CDPHP) supports roughly 400,000 members across 36 New York counties, including many rural areas with limited access to high-quality care. To extend timely, affordable care and remain competitive, CDPHP partnered with Included Health to offer Included Health’s Everyday & Urgent Care with Behavioral Health across all member populations.

Included Health implemented integrated virtual everyday and urgent care plus online therapy and psychiatry, giving members 24/7 on-demand access, directing them to the right site of care, and extending behavioral health into rural communities; CDPHP members have completed more than 14,000 behavioral health visits since 2021. The partnership also leveraged collaborative marketing and incentive programs to boost engagement, reduce costly ER and urgent care use, and improve clinical outcomes and member satisfaction.
