To empower its caregivers and standardize processes across its many clinics, HHMA partnered with vendor IBM to implement the IBM Phytel Patient Engagement suite.
The IBM Phytel solution provided automated patient outreach, comprehensive performance reporting, and tools for pre-visit planning. This implementation helped HHMA book appointments that generated approximately $2 million in revenue, dramatically improved screening rates by at least 10%, and put 87% of its clinics on track to achieve NCQA PCMH recognition, all while enabling a more patient-centered approach to care.
