The Polyclinic, a physician-owned multi-specialty clinic with fifteen locations and the first MSSP participant in Washington, needed to improve compliance with Medicare Annual Wellness Visit requirements after providers lacked consistent knowledge of eligibility and patients were confused by the “no-touch” visit format. To address those gaps, The Polyclinic engaged IBM Watson Health and its Population Health Management offerings (including IBM Phytel Outreach, Remind, Insight and Coordinate).

IBM helped The Polyclinic automate patient cohort identification, cross-reference payer data, deliver personalized multi-modal outreach, provide scripting for staff, and streamline scheduling so Annual Wellness Visits could be tacked on to existing appointments. As a result of IBM’s solution, the clinic scheduled more than 400 visits in a six-week pilot, completed 2,622 AWVs in 2015 and projected about 9,000 by the end of 2016 (a 95% year‑over‑year improvement), drove a $1.3 million increase in revenue in one year, and realized higher patient and staff satisfaction.
