The Patient-Centered Medical Home - Customer Case Study

The customer, a patient-centered medical home initiative comprising five healthcare organizations, faced the challenge of establishing themselves as fully functioning Patient-Centered Medical Homes (PCMHs) to improve care and qualify for financial incentives. They partnered with vendor IBM and utilized its Watson Health population health management solutions, including Phytel Outreach, Phytel Insight, and Phytel Coordinate, to integrate the necessary functions and attributes required for NCQA recognition.

IBM's solution provided the automated infrastructure for key tasks like data aggregation, patient outreach, care gap identification, and quality reporting. This enabled the healthcare providers to transition to a value-based care model. The results were significant, as all clients achieved PCMH recognition, with most practices reaching the highest NCQA Level 3. They also realized the benefits of the Triple Aim: better health outcomes, reduced costs, and improved patient satisfaction, with some organizations securing value-based contracts and additional payer reimbursements.
