The IBM Explorys Platform integrates disparate data sources to help identify at-risk patients

Mercy Health, a Cincinnati-based health system that operates more than 450 facilities and manages nearly 150,000 at-risk patients through Mercy Health Select, faced a challenge of mining disparate EHR and claims data to identify and act on rising patient risk quickly. To bridge gaps between different clinical systems and slow claims turnaround, Mercy Health turned to IBM and the IBM Explorys Platform (including the IBM Explorys EPM: Measure application).

IBM’s Explorys Platform integrated claims and clinical data, updated risk scores daily, and embedded patient summaries into clinicians’ EHR workflows so care managers could prioritize interventions. As a result, Mercy Health Select achieved a 97.1% ACO quality score in 2015—5.7 percentage points above the Medicare Shared Savings Program average—driving higher shared savings and faster identification of high-risk, high-cost patients.
