Prevea Health automates population health management and improves health outcomes

Prevea Health, a 180‑physician multispecialty group serving northeast Wisconsin with 15 patient‑centered medical homes and about 29,000 patients, needed an infrastructure to scale population health management and automate patient engagement as it moved from fee‑for‑service to value‑based care. Prevea selected IBM — specifically IBM Watson Health and the IBM Phytel suite (Phytel Coordinate, Phytel Outreach, Phytel Remind) — to integrate with its Epic EHR and replace manual registries and outreach processes.

IBM implemented the Watson Health/Phytel solution to identify care gaps, automate outreach, risk‑stratify patients and generate worklists and reports for care managers. Prevea achieved measurable impact: a 250% improvement in care‑management efficiency, 2–3x higher adherence for diabetes and hypertension, 207% more visits for contacted noncompliant diabetic patients and 124% more visits across contacted hypertension/diabetes patients, nearly a 50% drop in non‑adherence, improved routine blood‑pressure measurement (~96–99% in pilot clinics) and increased fee‑for‑service revenue while advancing toward value‑based care.
