Mercy Health, a premier healthcare provider in Ohio and Kentucky, faced the challenge of transitioning its 147 primary care offices to a Patient Centered Medical Home (PCMH) model within a strict 15-month NCQA certification timeline. Concurrently, they needed to integrate their Accountable Care Organization (ACO) and prepare for future bundled payment initiatives, which required an efficient way to manage clinical quality measures and glean insights from data across their entire network. To support these goals, Mercy Health engaged IBM and its Watson Health platform.

IBM implemented its cloud-based Explorys Platform, which provided the analytics and big data infrastructure needed for population health management. This solution enabled Mercy Health to track quality measures daily, rapidly train staff, and identify high-risk patients for proactive care. As a result, IBM helped Mercy Health successfully achieve NCQA PCMH certification, improve clinical outcomes, and reduce ER and hospital utilization, while also preparing the organization for future payment models.
