Arch Health Partners, a multi-specialty physician group in San Diego County, faced a challenge in improving hypertension control rates among its patient population, which were below the organization's standards. To address this, they initiated a population health management pilot using the IBM Watson Health platform to better identify and engage patients with uncontrolled high blood pressure.
By implementing IBM's solution, Arch Health Partners automated patient registry building and outreach, enabling a multi-disciplinary team to personally engage with at-risk patients. This approach, powered by the Watson Health platform, dramatically increased the number of patients with controlled hypertension from 63% to 77% in just six months, surpassing their annual goal. The program also contributed to a higher Medicare star rating and improved patient survey scores.
