IBM's solution enabled Utica Park Clinic to automate patient outreach and identify gaps in care. This initiative resulted in nearly 140,000 successful appointment reminders and informed 65,000 patients of care gaps. In a 60-provider pilot program alone, IBM's platform helped generate over $840,000 in additional billable revenue, achieving an extraordinary 14:1 return on investment and funding the PHM program's ongoing efforts.
