Floyd Health Care System, a regional health network serving over 350,000 patients, wanted to move from basic patient engagement to full population health management and achieve NCQA Patient‑Centered Medical Home (PCMH) recognition. To do that it sought to optimize its existing IBM Phytel solution from IBM Watson Health and better use the platform’s analytics and outreach capabilities.

IBM deployed a clinical application advisor and helped Floyd expand its IBM Phytel portfolio (Coordinate, Outreach, Remind, Population Health Management) to add previsit planning, care coordination and analytics-driven workflows. The IBM-led implementation drove measurable gains: up to a 41% increase in preventive cancer screenings in some offices (overall +22%), an average ~6% improvement in diabetic process measures across 10 offices (one office +18.6%), automatic credit toward PCMH criteria, and ultimately Level 3 NCQA PCMH recognition.
