A i2i Population Health Case Study
Golden Valley Health Centers (GVHC) is a community health system in central California that operates school-based clinics, women’s health centers, dental sites and homeless healthcare programs, providing care regardless of patients’ ability to pay. As part of the CDC/CMS Million Hearts® initiative, GVHC faced the challenge of shifting from missed opportunities to proactive identification and management of patients at risk for cardiovascular disease and uncontrolled hypertension.
By deploying the i2iTracks Population Health Management Platform, GVHC was able to screen, diagnose and manage high‑risk patients more quickly and efficiently; the platform’s analytics uncovered 1,513 patients needing hypertension screening, 805 were screened and 134 (17%) were diagnosed as hypertensive, enabling earlier intervention, better care coordination and measurable improvements in patient outreach and outcomes.
