Central Virginia Family Practitioners (CVFP) is a private group practice with eight primary care offices and two urgent care centers that set out to become a Patient-Centered Medical Home (PCMH). To meet NCQA PCMH requirements they needed to standardize workflows and extract detailed reports from their EHR, but quickly discovered the EHR alone could not support the reporting and improvement work.
CVFP implemented the i2iTracks Population Health Management Platform to track patient care, monitor outcomes and analyze results, enabling PCMH Level III recognition in 2014. Within six months they exceeded 10% improvement goals for breast and colon cancer screenings, pneumonia vaccinations, HbA1c testing for diabetes and hypertension follow-up visits, helping lower total cost of care and support ongoing quality improvement.
