Southeast Mississippi Rural Health Initiative, Inc. (SeMRHI) is a network of community health centers serving a predominantly rural region since 1979 across 16 clinics, 8 cities and 23 counties. Facing slow, cumbersome reporting for grant applications and NCQA-PCMH certification and lacking enterprise analytics, SeMRHI needed a faster way to access data and streamline population health management.
By implementing the i2i Tracks Population Health Management Platform, SeMRHI can now generate point‑and‑click reports without IT support, use the platform for grant and PCMH submissions, and quickly identify care gaps—finding 600 patients missing Healthy Use visits in minutes. The change improved outreach for immunizations and preventive care, accelerated grant workflows, and boosted ROI by enabling staff to focus more time on patient care.
