A i2i Population Health Case Study
Baldwin Family Health Care (BFHC), a federally funded community health center serving five primary care sites in Michigan since 1967, sought to improve outcomes for older patients with multiple chronic conditions who frequently used the emergency room. To become a Patient-Centered Medical Home and apply population health management techniques, BFHC needed technology to unlock data trapped in their EHR and generate actionable quality reports for staff.
BFHC implemented the i2i Tracks population health platform, which enabled unified data access, risk stratification, and large-scale outreach by care coordinators and case managers. Within 24 months the organization earned Level 3 NCQA-PCQH accreditation and significantly improved its ability to analyze clinical data, extend care reach, and increase the efficiency and impact of its care delivery.
