Home Health Care Management (HHCM), a home health provider serving eight counties, was struggling to track incidents, accidents, complaints, and other issues with a homegrown system that could only be used from the office. As a result, field caregivers had to call in or return to headquarters to file reports, which made the process slow, inconsistent, and difficult to use for trend analysis or compliance reporting.
HHCM implemented i-Sight to give clinicians an easy-to-use, configurable system with standardized workflows, built-in forms, and detailed reporting from the field. With i-Sight, incident resolution dropped from two weeks or more to 72 hours, routine reporting fell from an hour to five minutes, quarterly trend reporting was cut from eight hours to one, and field submissions increased fourfold. HHCM estimates i-Sight saves about $50,000 per year in wages and has improved compliance with Department of Health and Joint Commission requirements, with no citations for missing reports since implementation.
