Framework Outreach was commissioned to tackle entrenched and dispersed rough sleeping across Lincolnshire and Rutland, where local estimates showed 42 people sleeping rough on a single night and several hotspot areas. To build a countywide response, Framework Outreach received a three‑year Homelessness Transition Fund grant of £247,111 administered by Homeless Link.

Using Homeless Link funding, Framework Outreach set up a No Second Night Out–aligned street outreach team, a 24/7 freephone, rapid assessment and reconnection processes, and partnership working across local authorities. During the grant period the service supported 1,293 people, halved the single‑night rough‑sleeping estimate from 42 to 21 in year one, helped 123 people into accommodation in 2014/15 (82 into supported housing, 18 into private rented, 12 into long‑term local authority and 11 into temporary local authority housing), handled nearly 1,000 out‑of‑hours calls and completed 47 UK and 42 overseas reconnections; the Homeless Link‑funded model has secured ongoing local commissioning and public‑health support to sustain outreach.
