Shore Quality Partners, a Somers Point, NJ Clinically Integrated Network (CIN) with 240 physicians and a patient population of 40,000+, faced fragmented clinical, financial and operational data across multiple EHRs that made care coordination and ACO/shared-savings performance difficult. With HealthEC’s solution Shore reduced high-ED-utilizer spending by $1,600 PMPM, generated $4.69 million in 2016 shared savings across 16,000 patients, outperformed its geographic comparison group by $27.97 PMPM (about $5.37M annually), cut inpatient utilization 15%, increased primary care visits 3%, and maintained 90th-percentile patient satisfaction.
