Allegiance Health Group and HealthEC Deliver on the Promise of Accountable Care by Improving Quality and Reducing Cost

Allegiance Health Group, a 47-physician Accountable Care Organization in Trenton, NJ, needed a consolidated view of patient data across hospital EHRs, practice EMRs, claims and labs to identify high-risk, high-cost patients and improve quality while lowering costs. To address this, Allegiance partnered with HealthEC and deployed HealthEC’s population health management platform and consulting services.

HealthEC implemented an enterprise-wide solution to capture, normalize and analyze clinical, financial and operational data, enabling Allegiance to target the 5% of patients consuming nearly 35% of resources and to coordinate care more effectively. The HealthEC solution produced measurable results: SNF costs fell 33% per assigned beneficiary within 18 months; ER visits dropped 8% in 2015 and another 8% in 2016 (ER-to-hospitalization rates down 16% in 2015 and 10% in 2016); total inpatient costs decreased 6.25% in 2015 and 7% in 2016; short-term hospital stay costs per beneficiary fell 65% in 2015 and a further 10% in 2016; duplicate lab tests decreased 10%; ambulance use fell 32% after offering evening clinic hours; 30-day readmissions dropped 10% with a 5% increase in post-discharge visits; three care managers managing 800 patients drove about $700 average annual cost savings per patient, and Allegiance realized roughly $2 million (≈6%) in cost savings in 2015.
