U.S. Centers for Disease Control - Customer Case Study

The U.S. Centers for Disease Control faced rising healthcare costs driven by chronic conditions and an aging population, and needed a way to contain spend while improving employee wellbeing. They engaged HealthCheck360 to design a multi-year health and wellbeing strategy that included a health management program with onsite clinics, one-on-one coaching, health risk assessments, biometric screenings and participation incentives.

HealthCheck360 implemented onsite disease management and lifestyle coaching, ramped participation through incentives (73% in year two, more than 85% by year four), and shifted the plan to outcomes-based care. Over five years the HealthCheck360 program delivered $1,062,894 in overall health plan savings on a program cost of $387,320.33 (a 2.7:1 ROI), with $214,000 in screening-related claims savings and $340,000 saved by members managing chronic conditions — all at no additional cost to employees.
