Using A Data Warehouse and Analytics to Drive Population Health Management

Large Medical Center struggled for decades to compile and structure vast EHR data and establish a reliable baseline for value-based care and pay-for-performance (P4P) reporting. To address this, the organization engaged Health Catalyst and adopted its Late-Binding™ Data Warehouse platform, including the Health Catalyst Cohort Builder and the Population Health Advanced Application, to deliver near–real-time clinical analytics for population health management.

Health Catalyst implemented EDW-powered dashboards, integrated Patient Activation Measure (PAM) scores, and established 48 analytical measurements, enabling P4P incentive payment reporting and proactive care gap identification. Within eight weeks 10% of clinics were using the dashboards to manage about 2,300 patients, with a planned rollout to 100% of clinics covering an estimated 50,000+ patients; the solution provided near-real-time data, reduced manual reporting, improved preventive care and helped lower population health costs.
