OSF Healthcare, a Pioneer ACO with a long history in supportive and end-of-life care, faced the challenge of spreading advance care planning community-wide across a heterogeneous EHR environment. Health Catalyst’s EDW and IDEA enabled rapid data aggregation, risk stratification, near‑real‑time integration of advance care planning into the EHR, and automated reporting to drive adoption. Combined with training and community engagement, the solution helped OSF complete advance care planning for more than 16,000 patients, train over 980 physicians and facilitators, and exceed a target of 1,200 high‑risk patients (completing 1,243 in nine months), while increasing previously completed high‑risk plans by 64% and documenting over 4,300 non‑high‑risk plans annually.
