Healthcare System needed a better way to analyze and manage patients with chronic conditions, especially those at high risk for readmission. Facing value-based reimbursement pressure and CMS penalties for excess 30-day heart failure readmissions, it used Health Catalyst’s Key Process Analysis, Late-Binding Data Warehouse, Cohort Finder, and Population Health – Heart Failure Advanced Application to focus its improvement efforts on heart failure.

With Health Catalyst, the system built a multidisciplinary analytics and improvement program around four evidence-based interventions: medication reconciliation, post-discharge appointments, post-discharge phone calls, and teach-back. Eight months later, it reported a 29 percent reduction in 30-day heart failure readmissions, a 14 percent reduction in 90-day readmissions, a 120 percent increase in follow-up appointments, a 78 percent increase in medication reconciliation, an 87 percent increase in follow-up phone calls, and an 84 percent increase in teach-back interventions.
