Allina Health’s Courage Kenny Rehabilitation Institute (CKRI) needed to redesign rehabilitation care to focus on the whole person while managing rising costs and a shift to value‑based reimbursement. CKRI faced fragmented, siloed data and no single source of truth to identify high‑risk patients or coordinate care across inpatient, rehab, outpatient and community services. Health Catalyst implemented a late‑binding enterprise data warehouse and dashboards that give CKRI longitudinal, cross‑continuum views, early identification of patients with disabilities, and drill‑down functional assessments to guide targeted interventions and care coordination. Using Health Catalyst’s analytics, CKRI reduced hospitalizations by up to 76% and ED visits by 53%, achieved $11.2M in annual community cost savings, and can now measure outcomes and ROI for its programs more effectively.
